Original coin-op released by Data East in 1987. Ported to the NES in 1987 by Data East.

Sequels/spinoffs: A Street Fighter II-style fighting game called Karnov's Revenge was released as a coin-op in 1994 and was ported (as Fighter's History Dynamite in the US) to the Neo Geo, Neo Geo CD, and Saturn in 1994.

Karnov also appears a couple of times in Bad Dudes as a boss character and in the SNES version of Fighter's History.

Can anybody tell me what all the items do?

EDIT: I found out by reading the manual.

Summary:
Boots: jump higher for brief period of time
Bombs: kill enemies and blow up obstacles
Ladder: makes a ladder
Boomerang: use it to attack
Clapper: kills all enemies on screen
Glasses: see hidden objects
Swimming mask: swim faster
Wings: fly
Shield: protection
K: collect 50 for an extra life
