Was that one just a mock up shell or was there an actual proto?

It's an empty mock-up shell.

Also, can someone 100% confirm that Jerry had an empty Lady Bug shell too?

No, I've never had an empty Lady Bug shell. I have only Pepper II and a Carnival shells.

Pepper ii was never really started. I spoke with the programmer and he said it never got beyond the artwork and playfield stage.

Could you please make a better scan or picture for our database of that Pepper II shell?

IMO it's an interesting piece of video game history.
