This creates the 78 header (and signature) but I wanted a tool to read and maybe remove the 78 header to make the bin file.

So I created a tool with the output like this:

C:\roms>>readA78Headr.exe RAMPART.A78

I just threw this together, so feel free to change!
The Zip file has source and exe.
