I've got a 2600 cart that I couldn't find on the AA rarity guide. The label reads:

2 PAK SPECIAL
* CAVERN BLASTER
* CITY WAR

Anyway, City War is Atlantis, but I don't know what Cavern Blaster is.

The top part of the screen is the game field. You're a ship, flying through a cavern avoiding epilepsy giving walls, missiles, gun turrets and monsters (?). The bottom half of the screen is filled with bars which represent Energy, Shield, Time, and Score.

Anyone know what the real name of this game is?

Atarimania only keeps track of Holy Grails (3 or less known to exist).

That's R10 - R11 in your book.

As far as HES goes Pitfall II is the only one with a Holy Grail status (mainly because of the box):
