I recently purchased a copy of Indy 500 and I noticed that the cartridge label is slightly different than the one posted in the guide.

The differences that I notice are the copyright date is 1986, there is a black sticker over where it said "Use with Driving Controllers", and the picture is slightly different.

Does anyone else have this version?

That is a part of the 86' releases when the Tramiels took over .. They ussually came in a white box and had label mistakes often .. I would say they are a little rarer than the 82 type releases but nothinglike proto or quadrun rare .. there are a few listed here

The rarest Indy 500 item to get is the sears version of a complete boxed set(game box+driver controller box)<--Correct me if I'm wrong here.
The Tramiels sure could not keep things straight then;but made messed up label variants for us to collect later.
